Anyone can make a referral by completing our  Early Intervention  Program Request For Service  form (multi-service) or  Family Connections Program Request for Service form (behaviour support only).  You may also call us and ask to have the  appropriate Request for Service form mailed or faxed to you. Or you can drop in and pick one up at the Centre during regular business hours.

Children enrolled in a public school in Maple Ridge or Pitt Meadows may receive consultation services from a physical or occupational therapist through the School-Age Therapy Program. A referral for school-based consultation must be made by the school.
